Program that delivers home-based case management services to families of children with blood lead levels above 3.5mcg/dL and/or children with an established diagnosis of moderate to severe asthma. Case Management More Information: - Offers Environmental Health education and support for lead poisoning and/or asthma management - Provides help with identification and reduction of lead poisoning, asthma triggers, and other environmental hazards - Includes referral for lead abatement (removal) services at NO COST to the family/property owner - Provides cleaning supplies and pest management items throughout the duration of enrollment
